Documents required:
- Letter of Intent and filled up Application form
- Location map and technical description of proposed site
- Permits (Brgy., Municipal, DENR, etc.)
- Photocopy of DTI registration of Farm or SEC registration if under company name
- Photocopy of TIN Card
- Lease Agreement (if farm is rented)
- Performance bond (400 pesos/animal)

CASH BOND

Real Estate Mortgage (All Original Copies)
- Original copy of land title (TCT)
- Tax Declaration
- Tax Clearance (current year)
- Current Official Receipt of Real Property tax payment
- Affidavit of non-tenancy (if title is more than 1 hectare)
- Special Power of Attorney (if property is not in applicant's name)
- Photocopy of CTC (Community Tax Certificate)

Location must have:
- abundant source of potable WATER
- Reliable source of ELECTRICITY
- An all weather ROAD, for 6-wheeler trucks
- Good PEACE and ORDER
